RN Report Sheet
Sheet 1: Print this page (two-sided). Write Patient Room Number above the last name.
(I usually fold this sheet where only the last name and room number can be seen. Then, I place patient labels next to the corresponding last name).
Sheet 2: an Example of how to use the report sheet. All info on this page is made up. There is no real patient information on this page. Names, phone numbers, ages, diagnoses and histories are all fictional.
You'll get the report sheet that I developed to make organizing patient care, participating in rounds and giving report to the next shift efficient. When utilizing this report sheet, over time you'll get in the habit of quickly knowing where to find pertinent information because you will write it in the same place every shift. You will also know what questions to ask during report so that you can fill in the blanks in order to have a comprehensive look at your patient (on paper) which will help you become an expert at giving hand-off. Lastly, you'll be able to answer important questions when asked so, you don't have to use extra time signing onto the charting system to look it up. This report sheet is ideal for nurses who write small and believe in getting straight to the point when giving report.
